Kosteneffektivität als Allokationskriterium in der Gesundheitsversorgung
Confronted with steadily rising health-care costs, many countries have taken steps to set priorities in their health-care system. This paper examines the role that the cost-effectiveness of medical interventions could and should play in the allocation of scarce healthcare resources. After briefly introducing the underlying philosophical framework, we present different scenarios for the application of a cost-effectiveness approach in health care. We especially focus on formal economic methods like cost-effectiveness analyses. Finally, we describe the cost-effectiveness approach used in Oregon (USA) to develop a prioritised list of health services and discuss to what degree societal values concerning the allocation of scarce health-care resources can be included in cost-effectiveness analyses.
